I. Initial Survey Questionnaire

II. Ultrasound Examination (generally transvaginal ultrasound):

  1. Pre-Cycle Ultrasound for Uterine Assessment (not required for surrogacy clients)
    • Assessment of the uterus for fibroids and endometriosis.
    • The report should describe the size, shape, position, and endometrial thickness of the uterus.
  2. Pre-Cycle Ultrasound for Antral Follicle Count
    • Provide representative images of both ovaries.
    • Describe the size, shape, position, and presence of cysts or tumors in the ovaries.
    • Count the antral follicles in both ovaries, specifying the exact number on each side.

III. Hormonal Blood Tests (Day 3 labs): (to be done on the second or third day of menstruation)

  1. Follicle Stimulating Hormone (FSH)
  2. Luteinizing Hormone (LH)
  3. Estradiol (E2)
  4. Testosterone
  5. Progesterone
  6. Prolactin
    • (These six tests are generally referred to as the “sex hormone six-item panel” and are typically done together in most hospitals. They must be performed on the second or third day of menstruation. For example, if menstruation starts on the 1st, the blood test should be done on the 2nd or 3rd.)
  7. Anti-Mullerian Hormone (AMH)
  8. Vitamin D (VD)
  9. Thyroid Stimulating Hormone (TSH)

IV. Timing for Hormonal Blood Tests

  • These tests should be done on the second or third day of the menstrual cycle.
  • The transvaginal ultrasound can be done simultaneously with the hormonal blood tests or completed in the first half of the menstrual cycle.

V. Sexually Transmitted Infections (STI) Screening:

  1. HIV1/2 – Human Immunodeficiency Virus Antibodies Type I and II
  2. HBsAg – Hepatitis B Surface Antigen
  3. HBcAb – Hepatitis B Core Antibody
  4. Hepatitis C Antibody
  5. Syphilis Screening (RPR-FTA)
  6. Gonorrhea Screening (GC Aptima PCR) – can be done via urine or secretion sample
  7. Chlamydia Screening (Chlamydia Aptima PCR) – can be done via urine or secretion sample
  8. CMV IgM and IgG – Cytomegalovirus Antibodies
  9. Rubella Virus
  10. Varicella Virus
  • (Most of these tests are blood tests in China. However, Chlamydia and Gonorrhea tests are typically done with secretion samples in domestic hospitals.)
